Hmm, it's kinda shitty I'd need to swap out one of my good abilities for an ability that blows just to gtfo, but I'll look into it.
Teleport, vault, spirit walk. Commonly used GTFO abilities used by other classes. Many specs use 3-5 abilities just for survival and utility.
Honestly, I don't think the problems with melee and monks in particular have anything to do with the 3 points you made. Its a combination of these two things:
1. Nearly all incoming damage can be avoided through simple tactics like kiting and out-ranging by ranged characters while these tactics aren't naturally used or even possible by melee characters.
2. Defensive stat itemization progression is greatly outpaced by incoming damage in inferno.
I think the best possible (but least likely) fix would be to change the NPC AI and attack animations so that melee can develop their own tactics to avoid nearly all dmg while being just and productive as a kiting ranged class. This would probably be the only way to keep classes more or less balanced at all gear levels. Giving melee all the tools to kite like a ranged class turns them into a ranged class. Buffing defensive stat itemization enough to allow melee progression would trivialize the game at higher gear levels and make melee outpace ranged at higher gear levels.
